About the Session
The session will cover the basics of gender-affirming care in the primary care setting including, but not limited to, inclusive terminology and communication, how to create an inclusive practice environment, recommended guidelines for evidence-based care, specific considerations for gender diverse individuals in regards to screenings and family planning, and an introduction to the initiation and continuation of gender-affirming hormone therapy.

Why Attend?
Hospital at Home (HaH) is an innovative acute care delivery model within an alternate venue – the patient’s home. These programs have rapidly been adopted by healthcare institutions across the country since the Acute Hospital Care at home waiver by CMS in 2020. Literature on better patient outcomes and satisfaction, as well as decreased costs as compared to traditional brick and mortar hospitals has emerged since 2018. Hospitalists are at the forefront of providing care to eligible consenting patients in these models and must be knowledgeable of the history of HaH in the U.S., the different types of models that exist, the outcome metrics followed to track success and the current political landscape.
