ACP Practice Advisor® has released an updated module on managing diabetes and improving care delivery for patients with diabetes. The “Managing Diabetes Mellitus” module offers resources for clinicians to help identify key practice level interventions to optimize the delivery of care to patients with diabetes, motivate patients with diabetes to improve their self-management skills, and help practices deliver optimal diabetes care. Successful completion of this activity enables the participant to earn 20 CME and 20 MOC Practice Assessment points.
